Mental Health-Behavioral Health Provider (Integrated Care Therapist) Needed
  • FAMILY PRACTICE Office in Wartburg, TN
  • MCMC is searching for a full or part-time Mental Health-Behavioral Health Provider (Integrated Care Therapist) to join the BHI team.
  • Therapist is responsible for facilitating the integration of behavioral health into the primary care team to help
    Recognize, treat and manage psychosocial/behavioral problems and conditions. The position provides interventions directed toward the prevention, treatment, or management of physical health problems as they correlate with behavioral health concerns.
  • The therapist will provide direct clinical service to clients including: screenings, assessment, brief intervention,
    individual/family counseling, and possibly group counseling.
  • We are looking for someone who is comfortable working: with a wide range of ages and needs, as a member of a multi-disciplinary team, has experience with substance abuse treatment, willing to engage clients face to face in office and virtually.
Responsibilities include:
  • Provide comprehensive assessment and diagnosis of behavioral health patients.
  • Provide effective treatment planning to assist patients in successfully achieving goals.
  • Evaluate crisis situations and apply appropriate interventions (we do not provide mobile crisis or in-patient
    services).
  • Participate in meetings that support the integrated health care model to provide comprehensive patient care.
  • Assist in the detection of “at risk” patients and develop plans to prevent further deterioration.
  • Assist the primary care team in developing care management processes such as the use of guidelines, disease
    management techniques, case management, and patient education.
  • Provide assessment, consultation, and brief intervention for psychological/psychiatric problems,
    individual/family/couple counseling.
Requirements:
  • Education/Experience: Must be appropriately licensed or certified and have a master’s degree or Ph.D. in the
    behavioral/social sciences with a minimum of one year clinical experience.

  • Certificates and Licenses: Licensed in the State of TN as a Licensed Clinical Social Worker (LCSW), or as a
    Licensed Professional Counselor (LPC).

  • This position requires background check and drug screening. Valid TN driver’s license.
